Vancomycine iv

Remarks:

Orale step-down i.o.m. microbioloog/infectioloog. 

General comments

  • Na initiële IV-therapie, idealiter gedurende 1 week, overleg met arts-microbioloog en/of internist-infectioloog voor switch naar oraal regime. In geval van amoxicilline-resistente enterokokken: overleg over mogelijkheid linezolid per os onder monitoring van bijwerkingen.
  • Totale behandelduur 12 weken, tenzij prothese/kunstmateriaal geheel is verwijderd, dan 6 weken behandelen als osteomyelitis.

Algemene opmerkingen:

  • Vancomycine: bij voorkeur continu doseren, voorafgegaan door oplaaddosering.
  • Beta-lactam: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
  • Beoordeel gerapporteerde penicilline allergie aan de hand van de anamnese.

Zoektermen: PJI, kunstgewricht, FRI, fracture-related infection, fractuur, breuk, enterococ
