General comments
De geïnfecteerde shunt zo mogelijk verwijderen, anders externaliseren.
Bij voorkeur 7 dagen behandelen voor plaatsing nieuwe shunt.
Vancomycine: bij voorkeur continu doseren
Beta-lactam: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
Beoordeel gerapporteerde penicilline allergie aan de hand van de anamnese.
Indien ernstig zieke en goede nierfunctie: overweeg initiële dosering vancomycine te verhogen naar 3 dd 1000 mg.
