Verhoogd risico ESBL: bij hoge lokale prevalentie ESBL, infectie hospital-acquired/healthcare-associated, recent (< 2 maanden) gebruik breed-spectrum antibiotica, recent (< 3 maanden) reis naar hoog-endemisch gebied (zie https://resistancemap.cddep.org/) en recente (< 2 maanden) ziekenhuisopname in buitenland
Behandelduur: indien geen verwekker en focus is aangetoond en de patiënt goed opgeknapt is is een behandelduur van 7 dagen over het algemeen voldoende.
Bèta-lactam antibiotica: indien mogelijk continu doseren, voorafgegaan door oplaaddosering

* Let op bij gentamicine!

Geen gentamicine bij verminderde nierfunctie.
Er is een relatieve contra-indicatie voor gentamicine bij levercirrose.
Bij gentamicine moet 6-12 uur na eerste gift een midspiegel afgenomen worden. Indien het wordt gecontinueerd is controle van spiegels en nierfunctie noodzakelijk.
Gentamicine bij obesitas doseren op basis van aangepast lichaamsgewicht / Adjusted Body Weight.
