Bij antibiotische keuze rekening houden met kolonisatie/eerdere kweken.
Behandelduur: 5 dagen bij goede klinische respons. Bij onvoldoende respons behandelduur verlengen en diagnostiek naar empyeem of longabces verrichten.
Een HAP/VAP o.b.v. Pseudomonas aeruginosa 14 dagen behandelen (zie bron). Non-aeruginosa Pseudomonas species vallen buiten dit advies.
Bij aspiratie is het routinematig geven van antibiotica gericht tegen anaerobe bacteriën niet nodig, tenzij er een vermoeden is van empyeem of longabces.
Bij aspiratie bij adequaat gedecontamineerde patiënten op de IC zijn systemische antibiotica i.p. niet geïndiceerd.

Beta-lactam antibiotica: indien mogelijk continu doseren, voorafgegaan door oplaaddosering.
