General comments
E. dispar is apathogeen, alleen E. histolytica behandelen.
Behandeling amoebenabces of -dysenterie altijd gevolgd door contactamoebicide middel: clioquinol (suspensie FNA 100mg/ml) of paromomycine (tijdens zwangerschap). Paromomycine is in Nederland niet geregistreerd maar is een zogeheten "ZFW-middel”. Binnen de ZFW-middelen is paromomycine voor deze indicatie niet geregistreerd en wordt daarom niet vergoed.
Amoebenabces: zonodig: aspiratie. Aspiratie overwegen als diagnose onzeker is (o.a. serologie niet conclusief of niet beschikbaar) of bij dreigende perforatie. Voorspellend voor de behoefte aan aspiratie: 1) grootte abcessen > 10 cm; 2) betrokkenheid van beide lobben van de lever of abces in de linker lever kwab; 3) het falen van de medicamenteuze behandeling.
